구강 세정제

3. 종류 및 성분

구강 세정제는 작용 방식과 성분에 따라 다양하게 분류된다. 주로 구강 내를 살균하거나 구취를 억제하기 위해 사용된다. 사용 방법은 적당량을 입에 머금고 20~30초 정도 헹군 후 뱉어내는 것이 일반적이다. 칫솔로 양치질을 하기 전이나 후에 사용하거나, 소량의 구강 세정제를 입에 머금은 채로 양치질을 할 수도 있다. 제품 형태는 병, 스프레이, 1회용 포장 등 다양하다.

치약을 사용한 양치질만으로는 치아 틈새의 치주병균을 제거하기 어렵기 때문에, 구강 세정제가 치아 틈새까지 침투하여 도움을 준다. 물로 헹구면 효과가 떨어질 수 있으므로, 가글 후에는 헹구지 않는 것이 좋다.

바쁜 현대인들은 양치질 시간을 줄이기 위해 구강 세정제만 사용하는 경우가 있지만, 치태 제거를 위해서는 칫솔을 사용한 양치질이 반드시 필요하다.

일본에서는 약사법상 화장품, 의료용 의약품(의약부외품), 제3류 의약품으로 분류된다^[111]。포비돈 요오드나 불소가 배합된 구강 세정제는 의료용 의약품으로 후생노동성의 승인을 받아 치과 진료 등에 사용된다^[105]。

4. 사용법 및 주의사항

일반적으로 약 20–50ml의 구강 세정제를 사용하여 약 30초 동안 입안을 헹구거나 가글한 후 뱉어낸다.^[10][11][12] 대부분의 회사에서는 구강 세정제 사용 직후 물을 마시지 않도록 권장한다.

구강 세정제는 치약에서 남은 불소 잔류물을 씻어낼 수 있으므로, 양치질 직후에는 사용하지 않는 것이 좋다. 영국 국민 보건 서비스 캠페인에서는 칫솔질 후 "헹구지 말고 뱉으세요"라고 권고했다.^[13] 불소 구강 세정제는 양치질과 다른 시간에 사용할 수 있다.^[8]

대부분의 구강 세정제에는 에탄올이 포함되어 있어 입에 머금으면 자극이 있을 수 있다. 실수로 마시면 알코올에 약한 사람은 취할 수 있으며, 구강 세정제 사용 직후 운전하면 음주 단속에서 기준 이상의 알코올 농도가 검출될 우려가 있다. 따라서 최근에는 알코올을 사용하지 않는 논알코올 타입도 시판되고 있다.

자일리톨 가글은 세균 억제제 역할을 하며,^[76] 알코올로 인한 구강 건조를 피하기 위해 알코올 대체물로 사용되어 왔다.^[2]

5. 오용 및 남용

일부 브랜드의 구강 세정제에는 알코올 함량이 높고 섭취에 유해한 다른 물질이 포함되어 있어, 섭취 시 심각한 피해나 사망에 이를 수 있다.^[15] 알코올 0% 구강 세정제도 있으며, 다양한 요구에 맞는 많은 제형이 존재한다.

어린이나 어린이가 치약 및/또는 구강 세정제를 감독 없이 사용할 경우 삼킬 수 있어 이러한 위험은 더 높을 수 있다. 이러한 오용은 부모의 관리나 감독, 그리고 어린이용 구강 세정제를 사용함으로써 예방할 수 있다.

노숙인에게서 겨울철 사망의 흔한 원인인데, 구강 세정제를 마신 후 몸이 더 따뜻해진다고 느낄 수 있기 때문이다.^[16]

6. 논란 및 연구

일부 연구에서는 알코올 함유 구강 세정제가 구강암 위험을 증가시킬 수 있다는 주장이 제기되었으나, 명확한 인과 관계는 밝혀지지 않았다. 프로필렌글라이콜과 라우릴황산나트륨의 독성이 지적되기도 하지만, 구강 세정제에 사용되는 살균 및 화학 성분은 대부분 일반 치약에도 포함되어 있으며, 사용 농도를 고려할 때 건강상 큰 문제는 없다고 알려져 있다.^[39]

매일 2회 이상 구강 세정제를 사용하는 사람은 전혀 사용하지 않는 사람에 비해 당뇨병에 걸릴 위험이 55% 증가한다는 미국의 연구 결과가 있다.^[109] 이는 구강 세정제가 구강 내 유익균까지 살균하여 혈당 조절에 관여하는 세균이 줄어들기 때문으로 추정되지만, 정확한 기전은 아직 밝혀지지 않았다.

한편, 국립국제의료연구센터 연구에 따르면, 당뇨병과 치주 질환은 밀접한 관계가 있으며, 치주 질환 환자는 혈당 조절이 어려워져 당뇨병이 악화되기 쉽다.^[110] 일본치주병학회는 구강 세정제에 포함된 성분이 치주 질환의 원인균인 플라그를 억제하고, 치은염을 완화하는 효과가 있다고 밝혔다.^[111]

종합적으로 볼 때, 구강 세정제 사용과 당뇨병의 상관관계는 아직 명확하게 밝혀지지 않았으며, 추가적인 연구가 필요하다.
